Making a claim is simple with ShelterCare, just follow the steps below:
- Download and print out the .PDF of the claim form.
• ShelterCare Insurance Claim Form
• ShelterCare Gift Policy Claim Form
- Fill out Part 1 of the form. Be sure to print clearly and provide all required information.
- Take the form to your veterinarian and have them fill out Part 2 of the form and sign.
Note: Ask your veterinarian if they are using EVE (real-time claims filing for veterinary hospitals and clinics.)
- Sign and date your claim form before you fax or mail it in.

Just a few days after signing up with ShelterCare, "BK" and Indy were playing in the yard together and "BK" began to limp. We just thought "BK" had pulled or sprained his leg. However, when it did not get better, we visited our vet. X-rays revealed that his front leg bones had been dislocated and sent us to see a surgeon. Surgery was required to reset the leg bones, requiring screws to set the bones. Also, the main ligament around these leg bones had been stretched and the surgeon had to replace this ligament with an artificial one to stabilize and support the leg bones. There was a lot of aftercare required for "BK" including weekly x-rays, cast changes and related recovery expenses as well. We were quite amazed at ShelterCare's Service.
